Sarah Butler's initial on-screen experience came in 2004 as a contestant on the television program Love’s a Trip. Her acting debut followed in 2007 when she appeared in the film A Couple of White Chicks at the Hair Dresser as Marc’s assistant.
She gained widespread recognition in 2010 for her performance as Jennifer Hills in the controversial horror-thriller remake I Spit on Your Grave. This role significantly boosted her profile and remains one of her most notable performances. She reprised the character in I Spit on Your Grave III: Vengeance Is Mine in 2015, further establishing her place in the genre film community.
