After her elimination, she appeared in numerous shows promoting American Idol, including the annual "Idol" tour. She signed a record deal with 19 Recordings/BNA Records in 2006, and her first single, "Red High Heels," was released in the third quarter of that year. The single's music video was nominated for the CMT Video Awards for "Breakthrough Video of the Year." Her debut album, "Small Town Girl," followed in October 2006 and sold over 79,000 copies in its first week, entering the Billboard Hot 200 chart at number nine. The album includes songs about her struggles and was certified gold in January 2007. She was nominated for Top New Female Vocalist at the 42nd Country Music Awards and toured that year with country music star Brad Paisley, hitting at least 37 cities across North America and Canada. Pickler received a songwriter award at the 2007 ASCAP Awards for co-writing "Red High Heels." She had a memorable performance at the 2007 CMA Awards by singing "I Wonder," a song about her complicated relationship with her mother. She was distraught while singing and struggled to finish the piece because of her tears, and the performance received a long-standing ovation from the audience.

Pickler returned to the studio for her second album with the label 19 Recordings. "Don't You Know You're Beautiful" was picked as the lead single, and she performed at the ACM Awards in May 2008 and released the song a month later on the radio. She released a self-titled album in September 2008 and was compared to Jessica Simpson in a March 27 story in U.S. Weekly magazine titled "Kellie: The Next Jessica Simpson" due to her "sweet Southern style and ditzy demeanor." Pickler revealed yet another talented side of herself when she became a contestant on the 16th season of Dancing with the Stars, where she and partner Derek Hough wowed the judges with their dance routines. They won the competition, beating out actress Zendaya Coleman and Olympic athlete Aly Raisman.

Philanthropy

In late 2012, Pickler made news headlines for cutting off her long, blonde locks. She shaved her head to show support for her best friend, Summer Miller, who was about to begin treatment for breast cancer. Pickler actively supports St. Jude Children's Research Hospital, where she helps raise funds and visits kids. She also participates in charitable events to help raise funds for various organizations. She has performed on many shows to help raise funds for the "N.C. Children's PROMISE" to benefit the Children's Hospital. On November 12, 2006, she played a part in helping to refurbish the Hurricane Katrina-damaged NFL Youth Education Town Boys & Girls Club of New Orleans, and on December 16, 2006, she visited St. Jude Children's Research Hospital. On January 12, 2007, she visited Pennington Elementary School.
